  Initial Registration Form

If you are ordained as a deacon, priest, bishop or are a seminarian in the ordination process there are two ways to register with CDO:

1) Complete the Initial Registration Form online and electronically transmit it to CDO.   A Registration Kit will be mailed to you. A UserName and password will be emailed to you with a link to the CDO On Line System. Before completing your profile online, it is advisable to download: the On Line System User’s Manual, the Ministry Skill/Experience Manual and the Long Registration Form (as a worksheet) from CDO Forms.
2) If you order a Registration Kit in the mail, you will receive all three documents in 1) above.

1) Download Long Registration Form from CDO Forms.
2) Download and review Ministry Skills/Experience Manual
3) Follow the instructions and complete the Long Registration Form
4) Mail the completed form to: CDO, 815 Second Avenue, New York, NY 10017
5) Once we receive the Form, your Personal Profile will be created. You will also receive a User Name and password for accessing your profile online.
